ACM Care was started in 1999 as a nonprofit organization dedicated to improving the quality of life for frail elderly and disabled persons. We currently have three offices serving the local regions of St Louis, Peoria, and Chicago.

Our Mission: To improve the quality of life for frail, elderly and disabled people living in the community through case management and support services, regardless of their ability to pay.
